I can understand the man declining surgery at his age. I have an elderly aunt who declined treatment for her leukemia. But another elderly cousin went for surgery to remove part of her colon with cancer in it.

I'm inclined to think he's had cerebral atrophy for awhile, and the air is just filling the space. That is, the air is just taking up space that brain material used to.

There's no evidence of mass effect (the falx is still centered), and without the overpressure and consequent hypoxia there's no obvious mechanism for air-caused atrophy.

Yeah but the meninges that surround it do. A little bit of spinal fluid loss or inflammation is the worst headache you can image. There's a reason for the name spinal tap and post spinal/lumbar puncture headache.

That's Carlos "Halfy" Rodriguez, lost his head in a car accident after he went through the windscreen and hit the pavement. Head first I'm guessing. I doubt it was a slow enough progression, then again, he does frequently end up in the news for things like setting his own mattress on fire, so his functioning does indeed appear to be impaired, only it's no more impaired than the average Florida man.
