That chart is absolutely terrible. It looks like an abstract painting of the sea. No useful axis or label, and a set of useless instructions.

Try making your graphic more info, more usable, not pretty and useless. Jerks.

Well, I don't think there is any question about it. It can only be attributable to human error. This sort of thing has cropped up before, and it has always been due to human error.

The easiest password to guess is one that's so hard to remember that it ends up on a sticky note on the bottom of the keyboard.

First thing I thought of too. Social Engineering precedes the Internet. Hell, it precedes computers or even electricity.

"The Art of Deception" should be required reading for anyone even tangentially connected to handling sensitive information.

Back when Outlook viruses were all the rage you pretty much never looked at anything from corporate headquarters.  Not becasue it was mindless corporate dronespeak (everything *intentionally sent* certainly was), but becasue it was almost always a virus attempting to propagate.
/sending a patch to IT typically works
//sending a patch to a boss who thinks he understands computers *always* works
///but *of course* you can have to use Microsoft products
/V [note: badUSB is an attack that works on any USB system that obeys the standard completely.  No idea if Linux patches the USB to avoid that.  Still, even Ubuntu won't automatically run anything off a USB stick, you need some serious hacking to make a badUSB].

Even worse, the 'how to read' instructions are for a char that is upside down from the one posted.

As a pretty and useless jerk, I take offense to this.
