I've found an cheap replacement off eBay is better than any OOB charger.

connector may have gone bad. If you can, get a hold of a new power supply and see if it charges the battery. If not, your computer is boned unless you wanna take it apart. If it does charge, just need to replace the power supply/cord.

Not an apple, reseat the battery. If it's an apple, power down then charge.

One of the old laptops I use (has XP on it) needs to be plugged in at all times. Zero battery power.

actually, I think it's the laptop itself.  I have two Dell Latitude D620s that I bought from state government surplus.  One always worked better than the other, but I cracked the LCD on it somehow.  So I threw the hard drive and battery in the other and am using the same power cord, but now the battery doesn't charge and if the power cord falls out, the laptop immediately dies.  I need to take the LCD out of this one and install it in the other, but haven't gotten around to it.

Been there too. 
I've soldered a power receptacle in place twice on an old ThinkPad.

Batteries do wear out. I don't know where the info is on Windows, but the battery can tell you how much capacity it's lost. (acpitool on a Linux boot CD?)
