This is actually a bigger story than a simple engine failure.   See this AV Herald article.   The left engine cowl separated from the aircraft at the moment the plane rotated to take off.  The pilots leveled out at 6,000 feet, and were making plans to return to Heathrow when the RIGHT engine cowl separated from the aircraft.  That took a fuel or hydraulic line with it and started an uncontained fire on the outside of the engine that could not be extinguished because the cowl was gone.  The pilot brought the plane down safely before the fire spread to the fuel tanks and the plane blowed up.

Engine cowls do not come off both sides unless someone either forgets to tighten them after maintenance or someone purposefully opens them up.  Odds are it was a maintenance oops, but it would be easy enough to pass off as an oops if you worked in maintenance and had ill intent.
