I checked a map. East Palestine is on the border with Pennsylmania. It's not that far from Pittsburgh. While I doubt that many residents there are subscribed to that specific Twitter account, Pittsburgh media outlets likely are. They'll amplify the message. East Palestine does look to be in their coverage area.

Ya know how your phone will go off at any hour with an Amber Alert?  Those are based on the location of your phone at the time of the alert is sent.

East Palestine is in the Pittsburgh CWA, so the civil emergency message will be routed to mobile phones in the zip codes impacted by the evacuation order.   It's not weather but the NWS has the means to get the notifications out.

NATIONAL Weather Service. This is pretty smart- people will notification on their phones, weather apps, and smart devices, along with TV and radio in the area.

NOAA All-Hazards Radio is called "All-Hazards" because they relay warnings that come from both NWS sources and those that come from other governmental agencies, including Civil Emergency Messages and Child Abduction Warnings.

I used to live very close to there when I was a kid.

Youngstown does not issue weather warnings.  The national weather service is out of Cleveland and out of Pittsburgh... and as youngstown is closer to Pittsburgh, that area gets the weather alerts out of Pittsburgh (I still love the automated voice's pronunciation of Pittsburgh (Pitts-a-BURGH)... I wonder if it's still like that).

With that said, that area does NOT get Pittsburgh tv channels.  You can obviously tune in Pittsburgh radio stations (although some, like DVE, just barely) but do not receive local pittsburgh tv channels.  This was true even before digital became a thing and significantly shrunk the range.

/But I'm guessing this emergency alert is alerted in the same way that a weather alert would be... so they're probably just relaying the message as they would a weather alert.

Interestingly enough, the derailment happened less than a mile from the OH/PA border. So even if something happened in OH, it would most definitely be felt on the PA side of the line which does get all the local Pittsburgh TV channels in addition to the Youngstown ones.
