My mother worked as a hospice nurse for almost 40 years.  The for-profit firms did not have the best staff and often over-prescribed billable services the patients didn't need causing financial problems for the patients and their families.
So just another typical American health care thing.
The other major shift is that assisted living facilities are gone, all sold and converted into "at will" luxury senior living facilities with a la carte medical assistance services.  The average cost around here is $3000/month for a studio and three meals a day ($5000/month for a 2-BR).  Add on another $500/month for mobility and medication assistance.  Can't walk, other serious problems?  They evict you immediately.  It must be insanely profitable.
