No it was drugs. He was mixing codeine, morphine, quaaludes, valium, nordazepam, ethinamate, pentobarbital, pentobarbital and butabarbital. The only mystery is how he lived for as long as he did.

I loved the in-joke from Good Omens where Famine (the CEO of a fast food company that sells a product completely bereft of nutritional value) is doing a spot check at the franchise where Elvis is the fry cook and has him fired for singing on the job.

My wife had no idea why I was laughing so hard at that scene.  I didn't even try to explain the running joke from the book.

Yeah, but remember, in the 60s and 70s, doctors would prescribe amphetamines and quaaludes.
Having a doctor on 24-hour call offering every pill and shot in the bag to keep him performing is what did him in.

All of that while suffering from megacolon. If he wasn't constipated before the drugs, he certainly was after!

He was taking some of the pain meds because of the megacolon issues. His ego couldn't handle going around with a colonoscopy bag taped to his side.

However, the genetics of cardiac disease and a southern diet was going to kill him early anyway.

Alpha-1 Antitrypsin deficiency can kill you, but if he was just a carrier, that's damn unlikely at age 42. Carriers/heterozygotes can get liver disease and/or emphysema later in life, but homozygous/2 copies can and do die fairly young with some regularity. Just not of heart attacks on the toilet.

I've read he may also have had Hirschsprung's disease, which affects bowel enervation & can lead to disabling constipation, which combined with his substance abuse problems would've been extreme. But as far as the toxicology report, despite the variety of drugs he was using, only codeine was found to be elevated, and not at overdose levels.

At his autopsy, they found he had a toxic megacolon, so it's possible he had Chagas' disease, as well. I'm guessing there may have been kissing bugs in rural Mississippi in his time; they are definitely there now. Chagas' causes both heart disease and megacolon.

He died straining on the toilet while reading a book called "The Scientific Search for the Face of Jesus", and was found ass up with no signs of struggle. Dare I say the man was blessed. Prescription drugs, obesity, heart disease, and perhaps genetic and/or infectious disease all contributed to his death.

Difficult to believe that he was only 42 years old when he died.  I always thought he was at least a decade older than that, but, then again, I was just a kid at the time and every adult was "old".
