My guess is going to be ecoli in the leeks. Leeks are really, really hard to clean correctly and are basically the perfect place to hide contaminated soil.

Other suggestions I have seen:
- Aflatoxin contamination
- They accidentally used poisonous wild mushrooms instead of Porcini
- The lentils were undercooked (they naturally contain a protein which should not be eaten raw)

This Rolling Stone article gives more information about the symptoms that people were having after eating the product. If there is something wrong with the lentil and leek crumbles, it's some serious sh*t if it caused gallstones and liver damage in a week.
