Back in the 90s I tried to use my ATM card, no joy.  Tried 3-4 machines on the way to work, nada.  Turned out a co-worker had the same issue.

Turned out a blizzard on the east coast had caused a building to collapse.  The building that ran the ATM network.  Why didn't they have a backup?  Turned out that was the backup, the original location was in the World Trade Center, which had been bombed a week or two earlier.

I also remember a spate of stories from the 90s where a backhoe dug up an internet cable.  Why didn't they have a backup cable?  They did, but they ran it in the same pipe as the first cable.

Later in the article: Downdetector also saw a surge of reports from Mastercard users about issues using their cards as well - but the company later confirmed their network has been operating normally.

There was a computer at one of the mentioned companies that did frequent flyer miles. It was a big machine about the size of several refrigerators and was designed for two very large power plugs.  Whoever installed it was a bit confused and left a nice loop by the one corner of the machine.  The other plug wasn't installed.  Someone cut the corner a bit too close to the machine and pulled out its power plug. Opps.
