Could be a lot of things. Transplant recipients are highly encouraged to get counseling post-transplant.

Recipients are on lifelong medication post-transplant. The 1st year is extremely difficult. People who have near death experiences can really get mentally f*cked up. Depression is common. Feeling guilt for being alive is common. She could have been consciously or subconsciously lording it over him. Dunno.

Not excusing it. Just saying.

I think anyone who donates an organ should be mentally ready to accept that the receiptient can do or be as they please without obligation to anyone, same as the donor.  If you can't donate without that understanding, then don't donate an organ at all.
