So, it'll be just like here.  Doctors, nurses, scientists, and assorted very smart people will work 24 hour days on a very steep learning curve against this crisis.  Through their efforts, a true collapse will be averted.

Afterwards, stupid people who did nothing but contribute to the problem will say "See, they said it was gonna collapse and it didn't.  I told you it was no big deal."

Can't the US hire some paid trolls to just spam this (true) claim to Brazilian social media and incite a mob to give him to Mussolini treatment he deserves? I've heard here and there that anger at his vicious disregard for the mass death his actions are causing isn't *that* far short of revolt on its own.

I suspect you'd have to snow the patient just to get them to lie still for this. I know I wouldn't be able to tolerate that and I've helped intubate plenty of patients. Even with a clear knowledge of what they're doing and that it will be fine, I still don't think I could let someone do that to me. And if you're just securing everything to accomplish this (Clockwork Orange style) you're basically assaulting your patient.

I sincerely hope we aren't just sitting here with our thumbs up our ass and that we are, in fact, weaponizing Facebook and Twitter against Putin and Balls-in-a-row.  I'd it worked against us, it should work for us, and if we aren't doing it, we are stupid.

I just did an awake FO two days ago!  Only had to do about three in the last five or six years.  This last one was my best one yet, not a single cough!   It was for a case of angioedema, but we pulled the trigger in intubating early before things got too hairy.

I don't know why anybody still does lidocaine via cricothyroid membrane.  See all that coughing he did after the injection?  That's a potential laryngospasm and then a very bad time for everybody in the room.

Better to do nebulized 4 percent lidocaine.  Numbs things up nicely without the coughing.  Above the cords, you can spray a bit of 2 percent lidocaine before going through with the bronchoscope.

In any case, nobody is going to do an awake FO intubation on a hypoxic COVID patient.  That's not the indication for it.

And once intubated, people are sedated either way.  It maybe trickier to ventilate without paralytics I'd the patient is fighting the vent a lot, but by and large that's not always necessary.

What will likely be an issue is that they'll cancel elective procedures to save on drugs needed for intubation.  Or...anesthesiologists will go back to the old days with mask inductions, but that's just crazy.  They'll just cancel elective procedures instead.

A good awake fiber optic intubation is not, or should not be, distressing to anybody.

I'm not a big fan of the trans-cricoid injection of lidocaine.  That cough after is something I'd rather avoid.  I just do nebulized lidocaine, about 3ml worth, then I have them gargle and spit out the same stuff about twice, taking care to have them tilt their head and turn back and forth to cover everything.  Then, i use an atomizer to spray another 3ml to the base of the tongue and and back of the throat.  Once in I spray the cords from above as well.

Now it's time for the scope and you just advance slowly and give the patient a chance to tell you they're uncomfortable.  Last one I did I gave 1 mg of versed to take the edge off, and then just a whiff of a remifentanyl drip to further suppress the cough reflex.  Once the tube was in, I told him we were done and we're giving him medicine to fall asleep.  He nodded and gave me a thumbs up.

He got extubated today and I went to see him.  He doesn't remember the procedure at all.
