They move time zones all over the place for convenience. If it bothered the people on either island enough they might change zones. Nobody is enforcing any of it, each country can do whatever they want.
Heck, here in the US any state can do whatever they want, some states do, or did, let different parts decide.

Only the eastern one appears to be inhabited, and only by a small village. It looks like the western one used to have an indigenous population, but the Soviet Union moved them out in favor of a small military presence.

From what I can see on the map, there's absolutely nothing else on either island.

The village doesn't even have a port.  They have a helipad for the summer and carve an ice runway for bush planes in the winter.

If 24 time zones were equally distributed around the world they would be 23 hours apart

Big Diomede is in the right zone - Kamchatka Time - UTC+12
Little Diomede is merged into the Alaska zone 2 hours away - Alaska Time - UTC -9/-8

Friend of mine used to work oil in Alaska around 1980, and they'd occasionally cross the date line, at least when he was visiting Inuit friends.  The standard routine was you'd take a bottle of bourbon with you, which you'd trade with the Russian guard for two bottles of vodka and being allowed through, and give one bottle to the friends you've visiting.  On your way back, you'd trade a bottle of vodka with the American guard for two bottles of bourbon, one of which you'd keep around for the next trip.

Similar to being in space, there's no need for a time zone. So they pick one that's convenient for whatever reason - maybe to sync communication with ground or mainland.

Follow-up question.

So Superman reversed time by flying backwards and reversing the Earth's rotation. Could he have achieved the same thing by standing at the North pole and spinning around as fast as possible? Alternatively, could he have stood on his head at the South Pole, basically doing a break dancing move by spinning as fast as possible?

UTC. In fact we use Zulu, or Greenwich time all over and most science data is stamped in it as well.
