My read is that he was the new guy (so no one would have noticed he was missing), and on top of that he's some sort of space cadet or easily distracted, because how else do you get separated from the group THAT badly?

I got lost in a cave once.   It was an unmapped, unexplored cave out in the middle of the Nullarbor.

When we first went in there were two professionals there mapping it - lots of cords everywhere measuring distances.  We of course went much deeper and at some point I decided to explore a hole by myself.  I kept going and ultimately became lost.  Yes there was a moment of panic but luckily I found my way back to everyone else.

Been back out there a few time spelunking and have explored caves that no human had previously ever explored.

This is my daughter's caving club at IU (oddly, they don't like the term 'spelunking').  She was not on this trip, but knows the leader and the cave.  She said as soon as she heard it had happened, she thought it was probably the leader it turned out to be.  Apparently the kid that got left behind got separated from the group by going on ahead and went past the path to the exit.  Once he realized he was lost he stopped and waited for others to catch up, but they all went the correct way to the exit, so never caught up to him.  He backtracked, found the exit and everyone had gone and it was locked.  My daughter said they have a number of procedures to prevent this: head counts, buddy system, etc, that all failed because people were not paying attention.  The kid apparently went to class the next day, as well as his part time job, so he seems in good shape mentally and physically.  Could obviously have been much, much worse.

Hope that leader gets kicked out of that job, because that's just flat out incompetent. If his parents didn't push the issue of where he was they could have had a hell of a mess on their hands from not paying attention.

I was out there with my dad and my brother.  I found a hole that I could bearly fit through (dad couldn't.)  Coward that I was I lured my brother into the hole with me.  I am not even sure this cave has been recorded yet.  My brother and I keep going deeper and deeper, numerous points where we had to put our head sideways to get through.

Hours later we got back to the surface - it was twilight and we saw dad in the car with a map trying to find the closest emergency phone (oh we were in our twenties when this happened.)  It is one of our favourite stories.

Not downplaying that.  The kid that was trapped went to class and work the day after he was rescued.

I would think that the leader should be banned, and the person that was supposed to be his "buddy" should also be banned.

It shouldn't matter.  When you do activities like this you not only do a head count you do a role call before you lock the gates!  Also, why does a cave have gates?  Don't animals have a right to go in there?

Yea I'd figure that they'd do a role call (from a list and not memory because memory is spotty) both before they left the cave and before they locked up and left at the very least and for good measure even when they got back to the university or their homes via text message and make sure everyone was accounted.   In this case I'd assume anytime they stopped they'd do a role check as well to avoid someone being lost at any point and being able to find them easier (it's easier to find someone when you have a shorter window of time since you last saw them).

When I'm with a group of people I keep an eye out for everyone before we move on to the next thing, even the new guy/gal.  You better believe I do that before we actually leave for the night too.  I mean, it's basic group etiquette and safety. And it's so much more vital for a cave group to be concerned about doing and doing right than say my comic convention or Disneyland group.
