There are a few exceptions.  At the very least, NYC (596.7 vc/100k, 2014 data) is far bigger in population than Buffalo (1118.6 vc/100k).

State has only 1 city of any size => that city
State has a 1,000,000+ metropolitan area => that city

That pretty much covers 60% of the US.

Toledo for Ohio was a surprise though.  I was going to go with Cinci.

Los Angeles population: (2015 est) 3.9 million

Population 28,866 according to Wikipedia. I think the lists' cutoff was 50,000.

Not the whole state, just stay out of Toledo, Cincinnati, Youngstown, and Cleveland, and you should be fine.
