If people are so freaked out that hospitals or the government are collecting this data, my question is, why aren't you more freaked out that this data is available to collect?  It should bother you a lot less that the Hospital has this data, and more that ANYONE can buy this data on the open market from banks and credit card companies simply by asking.  For the right price I can get millions of detailed,fully identified  transaction and purchase history records from just about any major bank or credit card issuer

I want to stockpile chocolate donuts with preservatives, ho-hos, ice cream, frozen pizza, and a few cheesecakes. Then bring all of them to my next doctor's appointment and sit there with a cheshire cat smile on my face.  While balancing everything on my lap. Note to self.  Buy overalls, and a beanie cap to wear when doing this

They've been doing this a long time, albeit in a different fashion.  My wife's OB/GYN would tell her during her first pregnancy "Put down the ice cream, you getting to fat!"

Granted she put on 60lbs during that pregnancy.  She also lost it all and then some.

My company does this too. It flies under the guise of "wellness program", but they try like crazy to collect any data you are willing to give up in exchange for 20 bucks a month. This includes your diet, bad habits, blood work, baselines, etc. The company itself can't get specific data keyed to an individual because of HIPAA, but you wonder how honest these programs really are.

It's funny how people can read the same words and get two toatally different perspectives on what they mean. Where you see an overreaching, grandiose nazi conspiracy, I see medical care institutions being paid for quality of service instead of how many people they can herd through the current cattle car system.

To be fair, I think the Gravy Train was part of the problem. We put that stuff on everything. Once I froze and deep fried gravy and when the train came back around, I put more gravy on it. Yumm-o! Maybe if it only came around once every hour would've been better.

Totally disagree, when has the American Gov't ever created an institution that grew beyond it's original  scope? Answer: NEVER

When has gov't in America ever over reached it's authority? NEVER

When has money ever played a part in the way we receive health care? NEVER

I have to wholeheartedly agree with the people in this thread who all agree that there is nothing to worry about, the gov't has you covered and the gov't would never let anything bad happen to you. Look at the veterans, that's the single healthiest group of people on the planet.

No. They are not. This would cost hospitals a fortune for no return on investment. Hospitals and doctors don't want you to really live a healthy lifestyle. It isn't in their best interests for you to remain healthy. Insurers, would rather you pay big premiums and never use healthcare services, So, yeah, they may have incentive to mine big data for subscribers they can charge more money for not living healthfully. There's only one rub: while aggregating non-identifying data is fine, identifying individuals in such data collections, without the individuals expressed permission, is illegal. Revealing information about their healthcare is a HIPPA violation.

Just more anti-ACA propaganda and scare tactics. (as if there weren't enough bad things about the law to focus on)

Hospitals /= insurance providers. They won't be raising rates or denying coverage to anyone because they don't provide those things.

Really, you don't see a difference between questions and advice versus SURVEILLANCE

Here's an example:

Current: Doctor: it says on the questionnaire you filled out today that you are still smoking, I advise you quit.

Future: Our sources say that you purchased cigarettes and Ho-Hos last week. Twice. Also you have failed to do the required minutes on the treadmill. Recently, a drone spotted you sitting down and playing video games while eating Cheetos for 5 hours. Your Health Compliance score has been reduced to 54/100. You have passed the threshold of acceptance. We will no longer be monitoring at this point, but will be intervening. Your have been banned from purchasing the following food items. Well, since my alcoholic father lies to his doctor about drinking, his doctor can't understand why he's gaining weight and has high enzymes, this actually may save some people's lives.

My doctors have always asked me about my drinking, smoking, exercising and eating habits. And always told me to "Lose weight fatty". Sometimes the truth hurts, but his job is to keep me above ground so that's ok.
