We might have within a decade if the ACA didn't get passed, but now that healthcare is supposedly "reformed", there's no reason to go further. It'll be even worse if it fails because we'll just go back to how things used to be and no one will be willing to ever try that again.

/but mostly because the insurance companies won't allow it.

Even from just a strictly money perspective it's better than the current system in the USA.
Reduces GDP cost of healthcare by a large margin, and takes the burden of healthcare completely off employers.

The non-smoker discount is new and part of a overall company-wide effort to get people to quit smoking (I work in health care).

I did the math and by not being a smoker, I'm saving about $5/month on insurance next year vs. this year. Whereas if I was a smoker, my rates would be going up about $20/month.
