An accurate headline would read:
"An 18-year-old woman is dead after negligent driving resulted in a crash in a small town in western Michigan that also left her two passengers injured."

The reader still knows she didn't crash deliberately with the word negligent.

Accidents happen by chance with no apparent cause.  A tree falling on your car is an accident.  Skidding on black ice and hitting a guardrail is an accident.
Negligence is failure to take proper care in doing something.  Talking on your cell phone and applying makeup while driving resulting in a crash is negligent driving.  Running a red light and crashing in to a car is negligent driving.  Speeding recklessly on a dirt road in an attempt to jump a bridge is negligent driving.
