How could their lives, upbringing, experiences, etc. have so shaped their worldview in a manner that causes them outrage over this instead of shrugging their shoulders and going "meh" at the whole thing?

I would say Madonna was at least trying to do something artistic even if it was confrontational.

there was no message, boundary pushing, artistic merit with Miley.  Just a white girl who bought an outfit from Hot Topic/Forever 21 who can't dance.
