The speculation is that he is a past incarnation of The Doctor who did something so terrible that The Doctor no longer recognizes him as one of his incarnations (probably something to do with the Time War).  Hence:

Doctor:  He's me.  There's only me here, that's the point.  Now let's get back.
Clara:  But I never saw that one.  I saw all of you.  Eleven faces, all of them you.  You're the eleventh Doctor.
Doctor:  I said he was me, I never said he was the Doctor.
Clara:  I don't understand.

I think he's the same *person* as The Doctor, just someone who either renounced that (chosen) name, or didn't "live up" to it, possibly because of something he did during the Time War that wiped out all the Time Lords. If so, that would make Smith the twelfth regeneration, but still the eleventh "Doctor".

But I could be wrong; we'll find out on Thanksgiving.

I love Matt Smith's Doctor, but you know what I love more?

Change. The best part of "Doctor Who" is that it can change. I love to see what comes next, even if sometimes, what comes next sucks. At least it's different.

Yea, Steven Moffat can't od a good episode. Except: The Girl in the Fireplace, Empty Child, The Doctor Dances,  Blink, The Pandorica Opens, The Big Bang, A Christmas Carol, A Good Man Goes to War.

Do you remember how terrible the Russell T Davies  era was with this Jesus Doctor? Moffat isn't perfect but he's the best we've had since the show came back.

Can the new Doctor get two and three parters for most of his run? I'm enjoying the story setups, but they have to move too quick to fit in one episode, just go back to the old format from seasons 1-2 where we don't know when a two or even three parter is coming.

12th Doctor: Daniel Craig.  At the end of his run, the Tardis will explode, leaving the Doctor stranded in England sometime between 1962 and modern day.  The Doctor, being without the ability to travel through space and time, puts his genius to use and joins MI:6...  Whoever gets to be the 13th also inherits Craig's role as 007, settling once and for all that not only is James Bond a timelord, he is the actual Doctor.

It's always a shame when a Doctor decides to hang up the TARDIS keys.  Though we got some inconsistent episodes over the last year, Smith's run has been brilliant.

I do hope this doesn't begin a pattern of having the Doctor regenerate at Christmas, though.

That sucks, but you know, he's still on Trenzalore, and that's where we have the fall of the 11th, the TARDIS control room was that off the 11th's inside the Doctor's tomb, Clara already knows he regenerates, so it seems rather appropriate for this to be where Matt Smith's Doctor regenerates.

This gives me a sad. Probably my favorite Doctor since the relaunch. He brought the "alien" quality that was missing in David Tennant(who I still liked, but not as much as 11). Matt Smith's Doctor looked like a human(somewhat), talked like a human, but you could definitely tell that he was not a human. I really liked that part of the Doctor. Hope they keep up that aspect.

So, what are the odds the next Doctor will be a ginger? It's about high time they completed that running post-regeneration joke.
