Can and should.  Even if the Astros had held on, this might be one of the rate situations where a protest would be upheld.  If you really don't want that guy pitching, just have him get ejected.

I think a pitcher should need to pitch a full inning or 3 batters before being replaced.

I hate the Righty, Lefty, Righty 45 minute 8th inning where 4 batters are faced.

Oh, also pitchers get 3 warm-up pitches when they enter the game to gauge a feel for the mound.  They should be ready to pitch already.

Not a bad idea but how could that be accomplished?  He would have to throw a pitch I would imagine, giving the other team either a ball or a base runner on a hit batsman.  Not good in a close game.  Maybe just have him go batshiat on the mound, swearing at imaginary kids on his lawn and the ump.  Could result in a suspension and the buttstros pitch that Wright dude literally everyday thanks to their epically horrible starting pitching.

Not necessarily.  You could always attempt a pickoff then argue with the call.  Or just step off the mound and make disparaging remarks about the umpire and his mother.  If you were ok wasting a pitch, just throw a ball and then argue the call; that's an automatic ejection.  Is it possible there's some contingency for this very thing where the pitcher is required to complete the at bat, then is ejected?  Maybe, but I'd still like to see someone try it.
