Kwame Kilpatrick was a corrupt mayor of Detroit. He's a big, burly black man, but he's well-known for collecting Barbie dolls. He was using city funds to buy Barbies for his collection. So you can see how the public would pick this up and turn the Barbie dolls into Real dolls and next thing you know you've got yourself a meme.

It's pretty straightforward.
