Methinks that's the time to just let nature take its course. If the dog gets stuck for too long or things reach a critical juncture where he seems to be hurting Kara, maybe you could try pouring some cool water over the animal's schlong to see if that rectifies the situation. If the water calms him down, then his knot would shrink and he could pull it out of her.

That being said, I agree with zarberg that one of the freakiest things about this case is that the lady seemed unaware such an act would be considered so deviant. Either she was too wasted on drugs to give a damn (more than likely) or she's been doing it with dogs so many times that it's come to feel like something normal for her. Either way, whatta shame.
