Now, Carolina and Missouri? I farking loathe those two teams.

It's funny, I have the same reason for hating the dookies as most UK fans do, but it's actually because I was a huge Pitino fan at the time (you know, before he came to Boston and I lost most of the respect I built up by him bringing the Friars to the Final Four).  That and my aunt's family in NC are all Carolina fans (or lesser teams, like Appy, because he went there).  Between all that and having one of the most overrated color commentators EVER being dook's biggest fan, and it's actually not a hard thing to figure out why they're disliked.

Honestly, it's not unreasonable for any of the top four teams to have been #1. Inertia is probably what gave it to Duke, though.

There are some reasonable Kentucky fans out there. The problem is just the same as any team with a fanbase that's way bigger than the median- there's a LOT of room for idiots, and those people can be exceptionally vocal.
