I had periods about once every 12-18 months, so there was no way to use that particular metric to tell if I was pregnant.

Being pregnant (especially more than once) can cause discomfort. The main thing that gynecologists want to do with you if you say you're "uncomfortable" down there is jerk out your uterus. They tell you, somewhat enigmatically, that your "sex life will improve" and your husband will be happy.

Some women think this may not be completely true and that it's somewhat similar to a man having his scrotum removed. Some kind of loss of sensation and...well...something else.

At any rate, it's not surprising they jerked it out. That's what they do. It takes quite a fighting spirit to keep them from yanking it out, especially if you've got three kids (seriously, who needs four kids, boys or no boys?)

If a woman has uterine conditions such as fibroids, it may visually obscure an early term fetus on an ultrasound. That said, a stethescope should easily pick up a heartbeat of a baby that age.

2. They didn't do a pregnancy test, because the woman said she did one already and they took her at her word.
3. They didn't do a pregnancy test, because the woman said she didn't care if she was pregnant.
4. They didn't do a pregnancy test, and were totally shocked with what they found inside.

If #1, they're in the clear, because it was an honest error. If #2 or #3, then not so good for the hospital, but it might be okay if the woman signed an informed consent form, saying she understood the risks and accepted them. If #4, they're screwed. At least in America.

Again, gynecologists routinely offer hysterectomies to uncomfortable women without giving them "scans." They do a regular exam and suggest the operation.

Oh they do, I got sterilized at 24. Before my tubes got tied - pregnancy test. Since getting fixed:

Radiation therapy for the first round breast cancer - pregnancy test

Mastectomy for the second round of breast cancer- pregnancy test

Chemo therapy - pregnancy test

Reconstruction of my breast - pregnancy test

I'm pretty sure they gave her a pregnancy test, the test are not always accurate and sadly this one slipped though. Based on she was 14 weeks pregnant and didn't know, I'm assuming something weird was going on with the pregnancy that helped it escape detection. I had three failed pregnancies before getting sterilized and I always knew by 8 weeks, it's pretty hard not to notice you skipped your period for two months. But sometimes weird stuff happens, she might have still had a light period which may have led to it not being detected. It's very sad what happened but a person has to accept that medical science is not 100%. My cancer was not completely destroyed the first time around since some of it evaded detection for a period of time. It came back worse for round two, but I don't blame the hospital - they gave me the best care they could provide with the technology we have.
