I'm going to take a wild guess and assume that proper lockout/tagout procedure wasn't followed and that Jose was inside the machine when it was turned on.

Came here for this. I wonder if he was doing some maintenance and didn't lock it out. Some employee in a control room saw that oven #2 wasn't cooking fish, and turned it back on. Wasn't locked out, so it turned on with the poor guy inside. Even if is a quick 15 second job, you should be locking them out.

I've been told by people who did that sort of work that this was a horrible fate that daily hung over the heads of crewmen working in the engineering spaces of naval ships. One guy who was on the fire party of a ship I was on got to clean up after a steam leak that poached everyone in the space. He said that when they cracked open the hatch they found a mass of cooked bodies packed under it like sardines. Limbs came off when they tried to pull them out.

Quiet fellow, that.

I don't think that's trolling. Without OSHA keeping an eye on worker safety, many companies wouldn't concern themselves with it. Some would voluntarily care, but as history has shown us you might end up eating a lot of Soylent Green.
