To be eligible for LifeLine, you have to have an income below 135% of the poverty level or be enrolled in poverty programs (Medicaid, food stamps, section 8, TANF, etc). It is illegal for more than one member of a household to be receiving LifeLine benefits. LifeLine can either be chosen for a land line or a cell phone (but not both).

LifeLine is not a "free phone", but rather a discount of ~$10/month. Because of this, the "free" wireless plans are simply plans that are only worth about $10/month. 250 minutes/month on a shiatty phone, with texting cutting into that.

The phone itself is likely covered by Link-Up, a separate program designed to cover installation costs of landline phones and likely including initial purchase of cell phones.

The last makes up the majority of cost and effort in the USF.

Also, the USF is not paid with general tax dollars, but rather by mandatory contributions to the fund from telecommunication companies.

It looks like total spending is about $8.4 billion across all programs in 2010, and $8.7 billion in 2011.

That varies heavily based on demographic; in rural areas the amount of meth use seemed to have peaked at 40 parts per hundred, which corresponds to an increase in shiatty tattoos. In some areas, the male/female proportions have significant statistical variation, so you may be better served expanding your question to include tribal tramp stamps.
