The employer-based health insurance industry as we know it today arose because of government-mandated wage controls that prevented employers from paying employees more. It was part of the WWII price-fixing restrictions that didn't go away after the war ended. So, to entice employees, employers started paying employees extra with non-cash benefits, like health insurance.

Third-party payment (i.e., Isolating consumers from payment) is a major reason for the escalating costs.

It's just one example of how one government control leads to another, and another, and another ...

Given the way the court is going, particularly with their Citizens United ruling that went well beyond the arguments of the case itself, I wouldn't be shocked if they not only overturned HCR but also ruled that medical debts can no longer be discharged through bankruptancy.
