In the 1990's heroin got really popular in the Chicago Suburbs. All of the white kids from the far western suburbs use to drive to the West Side of Chicago to buy. The town I grew up in bordered the West Side of Chicago, so when they got done buying in the city they would find a place to park in my town and shoot up. For some reason the most popular spot for them to park and shoot up was this one particular McDonald's parking lot. The only problem was that is was next door to the village's police station. Everyday the police would get at least a dozen arrest for heroin possession just by simply walking the parking lot and checking the cars for passed out people or people shooting up in their cars. Heroin addicts aren't the smartest people.

That is not the only problem.

I once saw someone OD in a car at a public park at about 4:00 in the afternoon. I guess they thought tinted windows would keep people from seeing what they were up to. Maybe it even worked, until they started freaking out when their friend went unresponsive and they pulled him out and started beating on his chest. Hard not to notice that sort of thing. Must have ruined their buzz.

Gyrfalcon: I was always told you never ever slam Narcan. Titrate till the breathing improves, code 3 to the hospital and let the ER docs worry about it after that.

Well, think of it this way. How would you like to be woken up? Gently with a nudge, or having icewater thrown in your face.

Slamming NARCAN is like throwing ice water on that person. It's generally regarded as "Not being nice", as it immediately and completely removes any intoxicating effect of heroin for the next 15 - 30 minutes by blocking the Mu Opioid receptors and inactivating them. There's really no harm in doing so, although it has been known to make OD patients come up swinging. I learned of the glory of being such an asshole on my Paramedic clinicals, when I watched a Fire Medic do it to a regular Overdose patient.

I do exaggerate, though, just to seem like a bigger asshole than I am. I don't slam it. I usually titrate to the level of effect of semi-consciousness, just enough to keep their drive intact. I'm nicer than I try to put forth, and I don't really like wrestling with a pissed off junkie till I get to the ER

And the guy from Springfield would be one of the smarter residents from that city. People from the rest of Massachusetts get mad when people from the greater Boston area act like nothing exists outside of 495. But really, there isn't much to brag about out there, and people who know the area better than me have said it's like the redneck version of the Northeast. Appalachia, FTFY, just to be concise. Well, no. What you need to do is set up a system to produce opiates that are sold legally and given by prescription with very little oversight and lax standards of care, that are just as potent as heroin, then nobody will be breaking the law, just taking their medicine. Problem solved.

I just got my first felony doing something similar to this. Just started going to NA meetings. it's a hard drug to kick. I'd be surprised if I'm alive this time next year. It cost me everything in my life that was good.
