the Kaleds where the base creature whose mutation to adapt to the result of their prolonged war was accelerated by Davros in order to create his super weapon, the Daleks. since their forst creation they have continued to evolve and change, to a point where even their own technology stopped recognizing them. See "victory of the daleks episode" where some daleks where trying to activate an ark like device "the progenitor." unfortunately that device did not recognize them as daleks and refused to activate, not seeing them as "true daleks." so they satrted tweaking with earth's timeline waiting. sure enough the doctor shows up and decalres "I am the Doctor & you are the Daleks." progenitor then spits out the rainbow daleks.

They switched back and forth between Dr. Who and Doctor Who a few times before finally just settling on "The Doctor".

Even if "Doctor" is his name, names can still be abbreviated. Wm is an accepted version of William, for example. So even if I named my child Doctor it would still be acceptable to use "Dr".
