That's not bad photoshopping, just a terrible camera angle and arm positioning. The angle of the picture combines with the angle of her bent elbow to make her forearm seem to disappear

If that were true, the angle of the wrist and size of the hand wouldn't look totally wrong, and there wouldn't be such a sharp focus on both the hand and her body.

The first time I read the headline I read it as chopped and then I read the story and didn't get it.

The NYC Health dept (the folks who put out the poster) are well known for writing phoney ticket to New Yorkers hoping that people will pay them rather then go to court. When you go to court the officers who write the tickets either don't show up or admit to a mistake. They fired a bunch of people who wrote fake tickets a few years back. This doesn't surprise me.
