This happens about every other year in Portland, and we only have one MAX tunnel I can think of.

Alcohol doesn't necessarily have to be involved, a few years ago I was headed into work when the car in front of me made a abrupt left and fell off the road onto the tracks, I pulled over thinking it was a drunk who mistook the rail crossing for the road that ran parallel to it but it was a teenage girl who had been following her GPS directions and when the little voice told her "turn left now" she turned left NOW instead of 10 feet further on where the crossroad actually was, to make matters worse where she fell off at the road was raised above the tracks just enough that she couldn't back up onto the road, the rails were too high for her to turn and get off the tracks and the cross timbers were too high for her to go forward, so she was stuck on a active track at 715 in the morning and there is a Amtrak train that alwasy blows through at about 8 am, so I used my cell phone to call in the crossing number to let em know to hold the trains till a tow truck could get there, little miss GPS though was screaming and trying to grab my cell phone away saying not to call anyone that her daddy was on the way and would get her car off the tracks before anyone would notice. I managed to get through over top of her hysterics and found that the Amtrak wasn't scheduled that day instead there was a 730 AM coal carrier coming through, only 3 engines and 50 or so cars that they got stopped within sight of where we were, 20 minutes after the train stopped her daddy showed up, looked at how she had the car jammed in and called a tow truck.

It's Muni, the trains are ALWAYS slow moving.

It takes me 45 minutes to get 5 miles on the tram across the city (without changing lines) and 37 minutes to get to Palo Alto 30 miles south via Caltrain.

Muni is a joke. A halfway-house on wheels where diabeetus and/or English as a fourth language are jorb requirements.

A few months back, they mounted an EXTREMELY bright "DO NOT ENTER" sign at that tunnel entrance. It's so bright that it catches your eye during the day, and at night, you can see it from miles away. At the time, I thought, "Wow, people really need that kind of a reminder not to enter a light rail tunnel?" I guess the answer is not only "yes", but also, even that is not enough.

I work for a railroad (though no subway around here except for the sandwich shop), and this is quite common. And usually alcohol is involved. I have a few photos on Flickr, but work has the site blocked, so I can't link to or share.

I just came here to say this. I jog early in the morning, and I was up at Buena Vista park, and coming back down, and on the top of Duboce I could see it and thought it was an entire construction crew's lights.

One witness, Randall Gerstbacher, said the SUV had been behind a J-Church train as it headed into the inbound tunnel. The Metro train paused, Gerstbacher said, and the driver continued around it and into the outbound tunnel.
Muni employees in a nearby transit agency building chased the SUV into the tunnel on foot, Gerstbacher said.
