When you capitalize the words 'Spin Doctor' you are using them as pronoun.
When they are not capitalized as when the article calls someone a 'spin doctor' the words are simply nouns.

I, for one, thought the article would be about a former rocker biatching about England but your lack of grammatical knowledge led me astray.
