I remember reading The Life and Loves of a She-Devil where the main character got her legs shortened and it was pretty painful, so I imagine doing the opposite would be just as bad. Interestingly enough the main character got her legs shortened to emulate someone she despised, so it's like the direct opposite of this guy.

People can have a frame put around their legs, have their leg bones broken, then by using a screw on the frame, they gradually keep increasing the distance in the frame. The bone will eventually grow back together. Why yes, it is very painful.

Maybe this guy is doing it in one shot. I think the principle behind it would have to be the same though. And still really painful.
