The frail 80-year-old's death hours after drinking the fluid in July 2009 was easily avoidable,

But the death that he would have had anyways a day or two later was not easily avoidable. Unless you classify getting into a time machine and going back a decade or two in his life as easy.

That's horrible.

Unfortunatly, its basically also human error. Probably cleaning all the rooms, accidently left the bottle behind.
