Probably because the image is so gross. Personally I found The Shining (the book, not the stupid movie) very scary because he used a croquet mallet to bash people. No one really looks at croquet mallets as a murder weapon.

I think it's because of the implied brutality. It's technically possible to casually shoot someone, and the calmly-placed-knife-in-the-back is a classic movie villain move. It's harder to casually go after someone with an axe. Wild swinging and blood everywhere is kind of a prerequisite.
