My family's business makes BOP hoses, among other things. For about a week or so we didn't know whether it was our hose that had failed. That was fun.

sounds like some cockup failed to make a diagram

and who the fark modifies a friggin BOP control, the damn things are fairly standardized

and yes, i did work for a place that made them, ten years ago, oddly enough they had been sold off to Cameron about 5 years ago

Why is this the kind of device you would ever modify in the field anyway? I mean, it's really only designed to function one way, under one condition... did they add racing stripes and ground effects to it hoping it would go faster and look biatchin' while tearin' ass to the bottom of the ocean or something?

I doubt it was the BOP that was modified, most likely it was the controls

my best guess as to the BOP (new window)

and this is a variable bore ram packer (new window)

The BOP works like a large gate valve, where 2 rams are pushed towards each other, either to seal around the pipe or to shear it off and close it to contain well, what happened here

Every now and then a refinery accident will be the result of one person doing something colossally stupid. But those cases are few and far between. When your job can cause you to be blown up, set on fire, suffocated by toxic fumes or encased in molten sulfur, you tend to learn to be cautious pretty quickly.

The petroleum industry uses mainly standard measurements world-wide (at least at the equipment level).
