My widowed grandmother gets wedding invitations addressed Mrs. Arthur SoAndSo instead of Mrs. Irene SoAndSo. My grandfather has been dead for 20+ years. I think she would be offended if they dropped his name for hers.

we keep getting wedding invitations from mutual friends and they always put her name first on the envelope. it's like they don't realize that i'm the one that pays for the wedding gift and their pc bullshiat just downgraded them from a blender to a toaster.

I didn't take my husband's name when we got married, so we get things addresses either "Mr. and Mrs. [My Last Name]" or "Mr. and Mrs. [His Last Name]."

I think it would have to be "Dear Mr(s). and Mr(s)." Of course this assumes one took the others name, which is another sexist assumption (do the gays take one another's name when they marry? If not then any form would be exclusionary).
