He probably died in the water, after inhaling large quantities of it. First responders don't have to fill out as much paperwork if they perform CPR on the corpse long enough to let the doctors call it.

There used to be a kind of standard belief that a person resurfaces three times before fully drowning. (I don't know why anyone thought that, but whatever; that's what's being referenced.)
