I wonder what he thinks he looks like. As he kept paying doctors to tweak the structure and color of his face, he must have thought he was coming ever closer to some ideal image of himself. Or did he?

The surgery was within the realm of the normal until that radical change when his nose became very narrow and his skin became very white. It happened all at once. Did he see it as the same disaster as we do? Did he come out of that last operation horrified at what the doctors had done? Or in his mind, is that really what he wanted?

My guess is that the last operation was a medical catastrophe and didn't come out the way it was supposed to, and he knows it, but too much damange has been done and there isn't enough left to repair.
