I had a 24" cutoff blade on a 25 horsepower cutoff saw grenade itself at one point. 1/2 the blade went flying, which surprisingly nothing hit me, and the other half stayed on the machine that was still running. I jumped out of the way and then realized that I still had to cross over to the other side of the machine in order to shut it down. The 300+ pound saw was actually jumping up and down approximately 4 inches off of the floor. Needless to say, I took an unscheduled break after that was over.

Was it work related? I've heard about putting one's nose to the grindstone, but maybe he took it a little too literally.

those composite grinding discs like the one picture above are known to fracture and break apart due to improper use or defects. A blade spinning at a few thousand RPM turns a lot of angular velocity into linear velocity AKA those things have been known to kill people

I've had the smaller 3.5" cut-off wheels fracture on me but nothing more than a few knicks and cuts (remember safety glasses and face shield folks)
sounds like a face shield coulda saved him, but it's hard to know without the details

/that OSHA safety class was usefull afterall

Dave: Oh, man, it's totally serendipitous. Well, I got off work early, and you know my buddy Derrick? Well he was like, "I've got this acid, but I can't do it." And I was all like, "Well, I'll do it." So I did it. And by the time I got on my banana board, man, I was... I was tripping balls pretty hard, man. So I decided to get on my bench grinder, and a piece of metal flew up and hit me right in the eye. It was pretty awesome. And that brings us to now.

OK, so he can't see. The actual tissue damage isn't all that bad really. It will leave a scar, but they can do some neat stuff now. That and a glass eye, and he wont be that bad off. He wont be handsome, and he certainly wont be perfect, but he wont give people nightmares either.
