Probably a real doctor. Just picked a specialty with low malpractice insurance risk, treating problems the patients aren't going to die from, and probably aren't going to quickly recover from either. Like dermatology without the need to tell people they might have skin cancer.

A podiatrist does not have an MD title, no, though they are permitted to prescribe stuff and perform surgery to a limited extent. According to Wikipedia, dentists have the same deal going, at least in the US.
