That smock is a state-issued suicide smock. Its made of a single, heavy and reinforced-with-quilting material to prevent people who state they want to hurt themselves from doing so. IE ripping clothes up and hanging themselves with the strips.

That is a suicide prevention garment. Very tough to tear, rip, destroy. Held together with velcro so they cant use it to choke themselves.
