Unusual...the wrong baby is (sadly) quite common, and I'd imagine the wrong set of twins might happen, but one right and one wrong twin seems like an accomplishment - I would kind of expect them to keep the actual twins together, and that they'd be at least somewhat easy to distinguish even early on...
